Collecting information
To be able to deliver its services, PNZCT collects personal information including your:
- name
- contact information
- notes on interactions between us
Providing information is optional however if you choose not to enter your information, we may not be able to provide you with services.
From time to time, we may contact you for research purposes or to provide you with information about new products or services or special offers and promotions. This may include information about our programmes, research funding, special events, donor and fund-raising programmes.
Use and retention
We collect your personal information in order:
- to provide you with our products or services;
- to undertake customer service activities, such as responding to any queries, comments or complaints;
- to seek and receive donations;
- to interact with you about our events and activities;
- to process your applications for membership, to volunteer with us, and/or to become a client;
- to maintain and develop our business systems, including testing and upgrading them;
- for our administrative purposes and for internal record keeping;
- to comply with relevant laws and regulations; and
- for any other specific purpose which we notify you of at the time your personal information is collected.
We keep your information safe by storing it in a combination of secure computer and network storage facilities and paper-based files and other records. Only authorised staff can access the information.
Access to personal information and correction
You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong. If you’d like to ask for a copy of your information, or to have it corrected, please contact our Privacy Officer:
Sharing your information
Besides our staff, we may share your information when it is desirable to do so such as with our information system technology vendors or any contractors working with us who need such access to complete their contract.
Storage
Your personal information is stored in our Dedicated Client Management System.
Clients and Carers
A “Client” is a Person with a Parkinsonism condition (PwP) who has chosen to take advantage of the clinical services of PNZCT by signing the Client Consent Form.
A “Carer” means a person who is the primary carer of a PwP. It can include other family / whānau members who provide additional support and / or respite care.
Collection
We will collect your information directly from you and from other health professionals where you have authorised us to do so or where we are allowed to do so under the Health Information Privacy Code.
Use
As well as the uses mentioned under ‘Use and retention’ above we will only use your personal information:
- to provide our services to you and your family or whānau;
- to provide educational materials relating to living with Parkinsons; and
- to provide you with information about support groups and client specific events, activities and webinars
Sharing
We may share information with other health professionals involved in your care to facilitate the best possible care for you.
Access
Access to your personal health information is restricted on a ‘need to know’ basis to our clinical service staff involved in your care and necessary administrative support staff.
Members and Volunteers
A Member of PNZCT means a person, who has completed a membership application form (An Invitation to Support Us form) and paid the annual membership fee.
Volunteer - a person who donates his or her time to help PNZCT achieve its charitable purpose.
Collection
We will collect your information directly from you or for volunteers from a volunteer centre where you have authorised them to share this with us.
Use
As well as the uses mentioned under ‘Use and retention’ above we will only use your personal information:
- to engage with you as a member or volunteer;
- to add you to our national and regional newsletter mailing lists to enable us to communicate information about our activities and events; and
- to allow our regional committees to communicate with you about local activities and events;
Sharing
We may share your contact information with our regional committee in your locality in order to allow them to communicate with you about local activities and events.
Donors
A donor is a supporter who makes a financial contribution to PNZCT.
Collection
Your information will be collected directly from you or from any third-party fundraising provider you have authorised to share your information. As well as your contact information PNZCT collects and stores:
- notes on interactions between us;
- billing or purchase information;
- donation history;
Use
As well as the uses mentioned under ‘Use and retention’ above we will use your personal information to:
- interact and communicate with you as a prospective or current donor;
- send you mail with information about PNZCT and opportunities to contribute further unless you have opted out of receiving information from us by mail;
- send information to you by email if you have agreed to this; and
- to improve and enhance your experience as our donor.
